Important Information Regarding the Closure of the Arthritis and Osteoporosis Center

After 33 years of practicing rheumatology, Arthritis and Osteoporosis Center (AOC) will be closing as of April 1, 2026. At that time, I will no longer be seeing patients as rheumatologists. I will remain at Vivo Infusion in an administrative role.
It has been an honor to serve our community. I’ve been truly blessed and wish to express my heartfelt gratitude for the trust my patients have placed in me over the years. May god bless you and your loved ones with peace and good health in the years ahead.

When is AOC closing?
Arthritis and Osteoporosis Center will permanently close on April 1, 2026. After this date, we will no longer provide rheumatology clinic services.
Will my doctor still be practicing?
After April 1, 2026, Dr. Mawby will no longer be seeing patients as a rheumatologist. However, he will remain with Vivo Infusion in an administrative role.
Will I be able to schedule another appointment before closing?
We hope to see patients for at least one final visit prior to April 1, 2026. If you would like to schedule an appointment before closure, please contact our office as soon as possible.
What happens after my final visit?
Following your last appointment, your physician will no longer be able to provide ongoing rheumatologic care. However, limited emergency rheumatologic support will be available for 90 days after your final visit, or 90 days from the date of your notification letter if you do not have another appointment scheduled.
How do I continue my rheumatology care?
We encourage you to contact your primary care provider to discuss your ongoing rheumatologic care and obtain a referral to another rheumatologist if needed. Establishing care early will help prevent interruption in treatment.
How do I transfer my medical records?
Our staff will gladly assist with transferring your medical records to your new physician. To request records, complete a Medical Records Request Form or contact our office directly. Records will be securely transferred upon receipt of your signed authorization.
How do I submit a Medical Records Request Form?
The Medical Records Request Form can be downloaded by clicking here or requested in person. The completed form can be hand delivered or mailed to 418 Munson Ave, Traverse City, MI 49686 or faxed to (833) 957-2188.
How long will my records be maintained?
Medical records will be maintained in accordance with state and federal regulations. Instructions for requesting records will remain available.
What if I am receiving infusion therapy?
Infusion services through Vivo Infusion will continue to operate. If you are currently receiving infusion therapy, please contact Vivo Infusion directly to coordinate ongoing care.
What if I need medication refills?
Prescription refill requests will be addressed during the transition period. We strongly encourage you to establish care with a new rheumatologist promptly to ensure uninterrupted medication management.
